Transitional Living Program (TLP)

Description
The AFC Transitional Living Program (AFC-TLP) is also a new initiative funded by the Transitional Living Program grant (October 2017) to provide a safety net and strong emotional support system for young people who are homeless to make a successful transition into self-sufficient living. The program provides long-term residential services to homeless youth ages 16 to 22. Services are provided for up to 540 days, or in exceptional circumstances, up to 635 days.
The core outcome goals of the AFC-TLP are to provide youth with:
Stable housing: 90% of youth leaving a TLP will exit to a safe and stable destination
Permanent connections: 80% of youth will report that there is at least one adult in their life, other than RHY program staff, to whom they can go for advice or emotional support
Education/employment: 100% of youth in a TLP will engage in educational advancement, job attainment skills, or work activities
66% of youth leaving a TLP will attend school or will have graduated from high school or obtained a GED
75% of youth leaving a TLP will be employed or looking for work
